View or Print a Pre-Finalization Report

Before an adoption is finalized, a worker evaluates the adoptive family and adoptive children and produces the JFS01699 Pre-finalization Adoption Assessment Report, which is filed with the court at least 20 days before the finalization hearing. This report may include more than one child, but only if the children in the same adoptive placement home are being adopted together.

Once a pre-finalization assessment has been saved for a child, a new assessment record may not be created for that child, unless the child’s existing adoptive placement has been end-dated and a new placement record is created.

View a pre-finalization report as follows:

  1. From the Home screen, click the Case tab.

  2. Click the Workload tab. The Case Workload screen appears.

  3. Click the case ID Number link for the adoption case you want to view. The Case Overview screen appears.

  4. Click the Pre-Finalization Information link in the menu on the left. The Pre-Finalization Adoption Assessment Information screen appears. The screen displays only the children for whom an adoptive placement record has been completed.

  5. Click the View or Edit link in the Pre-Finalization Adoption Assessment table. The Pre-Finalization Report Details screen appears. It lists a series of links that open report sections.

  6. Click the Report link for the report section you want to view. The Document Details screen appears. If a report had previously been generated, and saved it will appear in the Document History field.

  7. Click Generate Report button. Select the radio button in the Version table.

  8. Click Generate Report button. The system generate the JFS 01699 in  PDF format.

  9. Click Save to save the document; or click Cancel. You are returned to the Pre-Finalization Adoption Assessment Information screen.
