Court Case Number

Sub-Type

Select the secondary type of motion. The items that appear in this drop-down list are a subset of  the selection in the Type of Motion field.

Date Submitted

Enter the date the motion was submitted to the court. Use the format MM/DD/YYYY.

File Stamp Date

Enter the court's file stamp date. Use the format MM/DD/YYYY. Once you enter and save this date, no further changes may be made to this motion record. You can, however, amend or end the motion.

Method of Motion

Select the method of motion. An oral motion is made during a court proceeding. A written motion is documented and filed with the court. This field is required.

Motion Supported by Affidavit

Select either Yes or No to indicate whether this motion is supported by an affidavit.
