Concern details (case plan) screen

This screen allows you to view, add, or update the detailed descriptions of family concerns, or risk contributors. These identify the needs the case plan should address in helping the family. A concern record must be associated with a service record.

The system automatically fills in the family strengths recorded on the family assessment. You can update these on the case plan as necessary. The case plan must include at least one strength and one concern.

How do I...

Fields on this screen

(Name check boxes)	Select the check box next to each case member to whom this concern applies.
Available Risk Contributors	Select the category(ies) of concern, then click Add>>. You can select more than one.
Selected Risk Contributors	This field displays the category(ies) of concern you selected in the list on the left. To remove a category, select it, then click <<Remove.
What is the concern?	Enter a detailed description of the concern(s) you selected above. You can enter up to 2000 characters. This field is required.
(narrative fields)	Enter a detailed answer to each of the questions displayed above the narrative text field. You can enter up to 2,000 characters in each field. You must answer every question before the case plan can be approved.
