Placement leave details screen

This screen allows you to view, add, or update the details of a child's leave from a placement.

Foster care maintenance, (FCM) reimbursements may be claimed if the eligible child is placed on leave from a family foster home; when a child is temporarily placed into another family foster home at the same level of care as the current family foster home for more than twenty-four hours but for no more than fourteen days and the child returns to the original family foster home by the end of the fourteen days.

Foster care maintenance reimbursement may be claimed for payments made to the provider to hold a bed while a child is in one of the following types of leave, if the leave does not exceed fourteen days and the child returns to the same placement he or she was placed in prior to the leave:

Fields on this screen

Leave Date	Enter the date on which the child's leave begins. Use the format MM/DD/YYYY. This field is required.
Return Date	Enter the date the child returned from the leave. Use the format MM/DD/YYYY.
Leave Reason	Select the reason why the child is leaving the placement. This field is required.
Estimated Return Date	Enter the date you expect the child to return from the leave. Use the format MM/DD/YYYY.
Location Type	Select type of location to which the child is going while on leave from the placement. This field is required.
Location Details	Enter a description of where the child is going while on leave from the placement. You can enter up to 2,000 characters.
