Placement request details screen

This screen allows you to view, add, or update the details of a placement request that is made before the child's placement. Any worker assigned to a case may record a placement request. The system sends a notification to the workers assigned to the case notifying them of the placement request.

The Link Provider button is enabled when you select the type of placement in the Service Type field and allows you to search for provider who match the criteria. When you link this placement request to a provider record, the system transfers information from the placement request record to the provider match screen.

When you open the placement record, the system transfers the service category, service type, child's name, and provider name from the placement request.

Fields on this screen

Responsible Caseworker

Select the name of the primary worker on this child's case. This field is required.

Responsible Placement Worker

Click Employee Search, then locate the worker who is responsible for placing the child, if different from the primary caseworker.

Placement with Siblings Preferred

Indicate whether efforts should be made to place this child with his or her siblings. This field is required.

Placement Needed Date

Enter the date on which the child should be placed. Use the format MM/DD/YYYY.

Estimated End Date

Enter the date on which you expect the child to be returned to the home. Use the format MM/DD/YYY.

Service Type

Select the type of placement service being provided. You must complete this service before you can link this placement request to a provider.

Status

Select the status of this placement request. This field is required.

When you set the status of the placement requests record to "Unable to Place" or "Request Withdrawn," the placement request record is frozen.

If the status is "Pending," "Request Completed," or "Placement in Progress," you cannot create a new placement request record for this child.

The system automatically changes the status of the placement request depending on the status of the placement record:

Placement Record	Placement Request Record
Created, in draft	Placement in Progress
Completed	Placement Completed
Deleted	Pending

Match Status

Select the status of the match between this provider and child.

Rejection Reason

If the match between this provider and this child was rejected, select the reason why.

Additional Comments

Enter any additional details concerning this placement request. You can enter up to 2,000 characters.
