Placement request search screen

This screen displays the placement requests for the cases in your workload. This log reflects the steps the worker takes to find and document potential matches.

You can refine your search by case ID, service type, placement w/siblings preferred, status, from date and to date. You can click the Edit link to edit a placement request record if it was created within your own agency or by ODJFS.

Fields on this screen

Child Name	Select child used for search function.
Status	If you want to view placement requests with a certain status, select the status.
From Request Date	To view placement requests in effect during a certain date range, enter the beginning date of the range. Use the format MM/DD/YYYY.
To Request Date	To view placement requests in effect during a certain date range, enter the ending date of the range. Use the format MM/DD/YYYY.
Sort Results By	Select the method by which you want the system to display the list.
Child Name	If you are adding a new placement request, select the child who the request is for. Click Add Placement Request button.
