Service Group

Select the service group you want to associate with this service record. This field is required.

Select Assessment/Investigation if no case plan exists. If a case plan does exist, select the item that best describes the case plan goal. See About case service groups.

Effective Date

Enter the date on which this service group is in effect. Use the format MM/DD/YYYY. You cannot enter a date in the future. This field is required.

This field defaults to the effective date of the case service record. You cannot enter a date that precedes this date.

If you add a new service group to this case service record, the effective date must be the same as or later than the last service group effective date.
