Select for Matching Conference

Select Yes or No to indicate whether this provider is being considered for a matching conference. If you select No, you must provide a reason in the next field.

Reason for Non-Consideration

If you selected No in the Select for Matching Conference field, select the reason why this provider is not being considered.

Decision Date

Enter the date you made the matching decision. This field is required if you selected Yes in the Consider for Matching Conference field. You cannot enter a date in the future.

Exclude from Future Matches

Select this check box if you want the system to exclude this provider when it performs automatic matching searches in the future. This field is enabled  when you select No in the Consider for Matching Conference  field. If you select this check box, you must complete the following field.

Additional Comments

Enter any additional information about the match between this provider and the child. This field is required you selected the Exclude from Future Matches check box. You can enter up to 3,000 characters.
