Adoptive family member information screen

This screen allows you to view or describe the composition of the adoptive family and the parents' marital information. The system draws this information from the most recent home study record. You can also add or remove family members from the JFS01699 report distribution using this screen.

Adding or removing family members here does not add or remove them from the provider record. All family members should be accurately recorded on the provider record.

If you add a mother or father on this screen, that parent is automatically added to the Adoptive Parent Description screen. Conversely, if you remove the mother or father on this screen, that member is also removed from the Adoptive Parent Description screen.

You cannot remove both a mother and father from the family composition described on this screen.

Fields on this screen

Marital Status	This field displays the marital status on the provider record of the adoptive parent(s). Select a different status if there has been a change in the marital status.
Marriage Date	This field displays the marriage date on the provider record of adoptive parents who are married. You can correct the date here if necessary. Use the format MM/DD/YYYY.
Place of Marriage	If you selected Married in the Marital Status field, enter the city where the adoptive parents were married. You can enter up to 40 characters.
Former Spouse's Name	If the mother or father was previously married, enter the first and last name of the former spouse. You can enter up to 40 characters.
Date Terminated	If the mother or father was previously married, enter the date on which the marriage to the person named in the previous field was ended. Use the format MM/DD/YYYY.
Mother Legal Resident	Select Yes or No to indicate whether the mother is a legal U.S. resident.
Citizen	Select Yes or No to indicate whether the mother is a U.S. citizen.
Father Legal Resident	Select Yes or No to indicate whether the father is a legal U.S. resident.
Citizen	Select Yes or No to indicate whether the father is a U.S. citizen
