Identifying information (case review/SAR) screen

This screen allows you to view, add, or update general details of the review, including the type of review, the case plan being reviewed, and the review participants.

The Select Available Case Plan(s) link displays a listing of the case plans recorded on this case. The case review is linked to a case plan in order to review the impact services are having on any safety threats, emerging danger, risk, permanency, and child well-being.

The Edit link opens the person profile for the participant. The Relationships link displays the relationship among the children in the case to the adult selected.

How do I...

Fields on this screen

Case Review Type	Select the type of review. This field is required.
If Optional Case Review, Explain	If you selected Optional in the Case Review Type field, enter a reason for the review. You can enter up to 200 characters.
Case Plan being Reviewed	Click Select Available Case Plan(s) to display the Available Case Plans to be Reviewed screen. Select the case plan, then click OK. This field is required.
Last Review Date	The system automatically enters the approval date from previous case review/SAR linked to the same case plan. If there is no previous review record, the field is blank. You cannot modify this field.
Last SAR Date	The system automatically enters the approval date from previous case review/SAR linked to the same case plan. If there is no previous review record, the field is blank. You cannot modify this field.
Sunset Activity	If this is an initial SAR, select the activity. If a previous SAR record exists, the system automatically displays the approval date of the last SAR for the associated case plan.
Sunset Date	If this is an initial SAR, enter one of the following, whichever comes first: (1) the date of the original filing for an extension or modification to an order for temporary custody or protective supervision, or (2) the date the child was placed.
