Service review details (case review/SAR) screen

This screen allows you to view, add, or update a service review for the services assigned to address the risk contributor selected on the Service Review Information screen. The table lists each case participant who is receiving the service. The check boxes next to each participant allow you to select the participants to which this service review applies. The edit link opens the Service Information screen, from which you can view or update the details of the service provided.

(Check boxes)	Select the check box next to each participant to whom this service review and recommendation applies.
Review Date	Select the date on which this service review was completed. Use the format MM/DD/YYYY. This field is required.
Service Recommendation	Select the action to be taken regarding this service. This field is required.
Additional Comments	Enter any explanation or comments related to the recommendation.
Do Barriers Exist?	Indicate whether there are barriers preventing progress. This field is required.
Barrier Type	If you indicated that barriers do exist, select the type of barrier.
If Yes, Explain	If you indicated that barriers do exist, enter an explanation of how this barrier is preventing progress toward addressing this risk contributor.
