ICPC/ICAMA child information screen

This screen allows you to view, update, or add information about a child's ICPC or ICAMA placement.

The Compact Termination tab is enabled only when the status of the ICPC/ICAMA record is "Approved."

Fields on this screen

Type	Indicate whether this is an ICPC or an ICAMA case. This field is required.
Sending/Receiving Agent	Select where the child came from or is being sent to. This field is required. This field defaults to the agency of the worker logged into Ohio SACWIS, as the receiving agent. If you represent the sending agency, select the state or entity receiving the child.
Agency Responsible for Planning for Child	Select the agency that is responsible for planning the child's care and placement.
Contact Person	Enter the name of the agency representative who should be contact about this case. You can enter up to 70 characters.
Phone	Enter the telephone number where the contact person can be reached.
Location	Enter the location of the agency. You can enter up to 100 characters.
Agency Responsible for Child	Enter the name of the agency financially responsible for the child.
Contact Person	Enter the name of the agency representative who should be contact about this case. You can enter up to 70 characters.
Phone	Enter the telephone number where the contact person can be reached.
Location	Enter the location of agency financially responsible for the child. You can enter up to 100 characters.
Agency/Person Child is to be Place with	Enter the name of the agency or the foster or adoptive parent this child is to be placed with. You can enter up to 100 characters.
Phone	Enter the telephone number of the agency or parent.
Location	Enter location where the child is being placed. You can enter up to 70 characters.
Provider Type	Select the type of provider the child is being placed with.
If Placement is for Adoption, the Adoption is to be Complete in	If this is an adoption case, enter the name of the agency that is handling the adoption.
Child's Legal Status	Select the child's legal status. This field is required when you select ICPC or COBRA in the Type field and you are a NON-ODJFS agency worker.
If Other, Explain	If you selected Other in the previous field, enter a explanation of the child's legal status. You can enter up to 250 characters.
