Initial report type (if applicable)	Enter the date for the item listed to the left of this field was sent to the out-of-state agency or received from the out-of-state agency.
Initial report type (if applicable)	Select the home study type from the drop down
Supervisory services	Select the type of supervision responsibility that was identified in the request. This field is required.
Supervisory reports	Select the frequency at which supervisory reports should be communicated. This field is required.
If Other, Explain	If you selected Other in the previous field, explain alternative methods and time periods for supervisory reports. You can enter up to 250 characters.
Date request for missing information was sent	Enter the date the agency sent a request for the missing information to the sending agency. Use the format MM/DD/YYYY. You cannot enter a date in the future.
Describe missing information	Detail the missing information in the narrative field
Date home study narrative sent/received	Enter the date the home study was sent/received
Date signed 100A sent/received	Enter date
Placement may be made	Document if the placement may be made
