Intake search

Date/Time Received

This section allows you to search for intakes that were recorded within a certain time frame. Enter the beginning date and time of the range in the From fields. Enter the ending date and time in the To fields. Use the format MM/DD/YYY for the dates and HH:MM for the time.

Intake ID

Enter the ID number of the intake record you want to view. (If you do not have this number, enter other search criteria.)

Emergency Intakes Only

To search only emergency records, select this check box.

Intake Category

To narrow your search to a certain category, select the category.

Intake Priority

To narrow your search to a certain priority level, select the priority.

Intake Type

To narrow your search to a certain intake type, select the type.

Agency

To search intake records for a certain agency, select the agency.

Intake Status

To narrow your search to a certain status, select the status.

Reporter Name

To search intakes from a certain reporter, enter the reporter's name. You can enter up to 40 characters.

Screener Last Name

To search intakes screened by a certain worker, enter the screener's last name. You can enter up to 25 characters.

First Name

To search intakes screened by a certain worker, enter the screener's first name. You can enter up to 25 characters.

Intake Part. Last Name

To search intakes with a certain participant, enter the participant's last name. You can enter up to 25 characters.

First Name

To search intakes with a certain participant, enter the participant's first name. You can enter up to 25 characters.

Address

To search intakes with the address that is associated with an intake participant, enter the first line of the street address. You can enter up to 50 characters in each field.

City

To search intakes with a certain address, enter the city. You can enter up to 30 characters.

State

To search intakes with a certain address, select the state.

Zip

To search intakes with a certain address, enter the zip code.

Sounds Like

Select this check box if you want the system to search street names that sound like the one you entered in the address.

Case ID

To search intakes that are associated with a case ID, enter the case ID number.

Case Last Name

To search intakes associated with a certain case, enter the last name on the case. You can enter up to 25 characters.

First Name

To search intakes associated with a certain case, enter the first name on the case. You can enter up to25 characters.

Sort Results By

Select the method by which you want the system to display the search results.
