Edit characteristics screen

This screen allows you to edit a person's characteristics description. You can edit the method, who they were reported by; end date them, or edit comments.

Fields on this screen

Characteristic	Select the characteristic you observed or the person reported. This field is required.
Subcategory	Select a subcategory that further defines the characteristics. This field is required.
Self Reported	Select Yes or No to indicate whether the person told you about this characteristic.
Observed	Select Yes or No to indicate whether this characteristic was observed.
Observed By	If the characteristic was observed, enter the name of the person who observed it. This field is required when you select Yes in the Observed field.
Observation Date	Enter the date on which the person observed this characteristic. Use the format MM/DD/YYYY. This field is required when you select Yes in the Observed field.
Clinically Diagnosed	Select Yes or No to indicate whether this characteristic was diagnosed by a mental or health care professional.
Diagnosed By	Enter the name of the professional who diagnosed the characteristic. This field is required when you select Yes in the Diagnosed By field.
Diagnosis Date	Enter the date the person was diagnosed. Use the format MM/DD/YYYY. This field is required when you select Yes in the Diagnosed By field.
Additional Information	Enter any further description about the characteristic, its observation, or its diagnosis. You can enter up to 2,000 characters.
