Multi-factor evaluation (MFE) history screen

This screen allows you to view, add, or update details about child in special education, including Multi-Factor Evaluation (MFE) and Individual Education Plan (IEP) records.

The check boxes that appear on this screen are read-only. The system selects boxes based on the characteristics selected on the Self Reported/Observed Characteristics and Characteristics Detail screens of the person profile.

Fields on this screen

Child eligible to enroll in special education	Select this check box if the child is eligible for special education but not yet enrolled in a program.
Child enrolled in special education	Select this check box if the child is currently enrolled in a special education program.
Enrollment Date	If the child is currently enrolled, enter the date on which the child was enrolled in special education. Use the format MM/DD/YYYY.
Achievement Level with Special Education	Select the level of achievement attained with special education.
Explain	Describe the child's needs for special education or achievement. You can enter up to 3,000 characters. This field is required if you selected one of the check boxes above.
Explain	Describe the child's needs and barriers in more detail. You can enter up to 3,000 characters. This field is required if you selected any of the check boxes above.
Developmentally Delayed	This field displays the characteristics defined in the child's person profile.
Mental Retardation	This field displays the characteristics defined in the child's person profile.
Explain	Explain the child's functioning in greater detail. You can enter up to 3,000 characters. This field is required if the child is developmentally delayed, mentally retarded, or academically behind.
