Copy of IEP Record

Select this check box if the agency has a copy of the child's IEP record.

Originating School District

Select the school district that initiated  the IEP. This field is required.

Other

If you selected Other in the Originating School District field, enter the name of the school district. This field is required when you select Other.

Effective Date of IEP

Enter the date on which the IEP was initiated. Use the format MM/DD/YYYY.

Expiration Date of IEP

Enter the date on which the IEP was longer in effect. Use the format MM/DD/YYYY.

IEP Participants

Enter the names of the people or organizations that are participating in this IEP. You can enter up to 2,000 characters.
