Provider Name	Click Search to locate the provider in the system. Health care providers are added through the Provider area.
First Visit	Enter the date the person first started seeing this health care provider. Use the format MM/DD/YYYY. This field is required.
Provider End Date	If the person stopped seeing this health care provider, enter the end date. Use the format MM/DD/YYYY.
