This screen allows you to view, add, or update the provider's address(es) and the provider's contact information.
You can update address information for a non-ODJFS provider from this screen at any time. For a home provider, you can update this information when the provider type status is "Application Received" or "Pending Home Study." For a residential provider, you can update the address information when a pending agency certification record exists. You can update provider information only if you are assigned to that provider. (See About provider information.)
The View Address History link displays a history of address changes for this provider. The Add Address and Add Contact buttons, when enabled, allow you to record a new address or contact person, while the Edit links allow you to correct or end-date existing records.
When a change is made to the address, the system sends a notification to all the workers assigned to this provider and to workers whose clients receive services from this provider.
