Provider address details

Address Type

Select the kind of address this is. This field is required.

You can record multiple addresses for a single provider, but only one address of a certain type may be in effect at a time. The previous address must be end-dated (manually or automatically) before a new address of that type can be recorded.

Primary Address

Select this check box if this is the provider's main address. This field is enabled if this is a home provider and the address type is Home.

A provider may have only one primary address. If another address is selected as the primary address, the system automatically clears the previous one.

The primary address may not be deleted if it was entered in error. Select another primary address before deleting the incorrect address.

Updates to the primary address are reflected on the address records of all household members.

Effective Date

Enter the date on which this address information was in effect. Use the format MM/DD/YYYY. This field is required. You cannot enter a date in the future.

End Date

If this address is no longer current, enter an end date. Use the format MM/DD/YYYY. You cannot enter a date in the future.

You can edit the end date only of the most recent record of this address type.  

The system automatically end-dates an address record if you record a new address of the same type.

C/O

If correspondence to this address is sent in care of a particular person, enter that person's name. You can enter up to 100 characters.

Delivery Details

Enter any special details concerning the delivery of packages or correspondence to this address. You can enter up to 250 characters.
