Effective Date

Enter the date this training requirement is effective. Use the format MM/DD/YYYY. This field is required.

Training Type

Select the type of training this member must complete. This field is required.

Level of Care

Select the level of care for which this provider intends to become, or is, certified. This field is required.

Training Status

Select the status of this training record, to indicate whether the member completed the competencies selected. This field is required.

Waiver Reason

If Waiver was selected in the Training Status field, select the reason why the training requirement was waived.

Available Topics

Select the competencies this member must complete, then click Add>>.

Selected Topics

This field displays the competencies selected from the list box on the left. To remove a competency, select it, then click <<Remove.
