Service Category

Select the category of service. This list displays the categories the apply to this provider type. This field is required.

If you are reopening a home provider record and adding services, select Placement.

Service Type

Select the type of service. This list displays the types that apply to the provider type and to the category you selected. This field is required.

Service Status

Select the status of the service. This field is required.

Service Capacity

If the service involves a number of participants at a time, for example, a group counseling or a tutoring session, enter the maximum number of participants allowed in one session.

Effective Date

Enter the date on which the provider began offering this service. This field is required.

If you selected Placement in the Service Category field, this date must be the same as or earlier than the date the child is placed with this provider.
