Contract details screen

This screen allows you to view, add, or update the details of a provider's contract to provider certain services for this agency.

The View and Edit links allow you to open a specific record. You can edit a contract when the status is "Pending," "Active," or "Payment Hold."

The Link Activity button displays the Activity Log list screen, allowing you to create a new activity log record that is linked to this contract. The Contract Activity area displays the activity log records that are currently linked to the contract. The View link in this area opens the activity log record, while the Edit link allows the worker who created the record (and this person's supervisor) to update the record. The Report link allows you to print the activity log record. The Unlink link removes the association between the service contract and activity log records.

The Amend link allows you to attach certain updates to a contract in "Active" status. The Add Amendment button allows you to amend the dates of the contract.

Fields on this screen

Vendor Number	If this service provider has a vendor number, enter the number.
Contract Begin Date	Enter the beginning date of the contract. Use the format MM/DD/YYYY. This field is required.
Contract Type	Select the type of service contract. This field is required. When you select "Placement," the Add Non-Placement Service Cost button is disabled. When you select "Non-Placement," the Add Placement Service Costs button is disabled. Both buttons are enabled when you select "Managed Care."
Contract End Date	Enter the date on which this contract period ends. Use the format MM/DD/YYYY. This field is required.
Contract Number	Enter the contract number. You can enter alphabetic or numeric characters.
Original Contract Amount	Enter the total amount of the contract. This field is disabled when the contract is in "Active" status.
Approved Date	Enter the date on which the contract was approved. Use the format MM/DD/YYYY.
Encumbered Amount	Enter the encumbered amount.
Encumbered Date	Enter the encumbered date.
Notification Type	Select the notification type. When a notification (contract amount or encumbered amount) is selected, along with a ceiling value (for example, 20%), the system multiplies the selected amount by the selected percentage and matches the result against the used amount to determine whether the ceiling threshold for that notification has been reached. If so, the system generates a notification to the contract administrator for the agency.
Notification Ceiling	Select ceiling. (Refer to the Notification Type field above.)
Status	Select the status. This field is required. If the status is currently "Active," you can change the status to "Payment Hold," Cancelled," or "Terminated." The system changes the status to "Expired" when the end date passes. This field is required.
Comments	Enter any additional information about this contract. You can enter up to 4,000 characters.
