Provider search

Provider Name

To search by the provider name, enter the name. You can enter up to 35 characters.

Provider Category

To include the provider category in your search criteria, select the category.

Provider Type

To include the provider type in your search criteria, select the type.

Agency Type

To include the agency type in your search criteria, select the type.

Agency

To include the agency name in your search criteria, select the agency.

Member First Name

To include the name of a provider member in your search, enter the person's first name. You can enter up to 70 characters.

Member Last Name

To include the name of a provider member in your search, enter the person's last name. You can enter up to 70 characters.

Sounds Like

Select this check box if you want to the system to look for last names that sound like the one you entered. You cannot select this box and use a wildcard (%) in the last name field.

Provider Reference Type

To search by another identifier, select the reference type.

Provider Reference #

To search by another identifier, enter the ID number.

Provider ID

To search by the provider's ID, enter the number. If you complete this field, you cannot use any other search criteria.

Street Number

To search by the provider's address, enter the street number. You can enter up to 10 characters.

Street Name

Enter the street name. You can enter up to 50 characters.

City

Enter the name of city. You can enter up to 50 characters.

State

Select the name of the state.

Zip Code

Enter the zip code, including the zip+4 digits, if you have them.

Sounds Like

Select this check box if you want the system to search for street and city names that sound like the one you entered.
