Home study details

Home Study Type

Select the type of home study you are recording. If you are adding an initial home study, the system displays "Initial." This field is required.

Assessor

Select the name of the worker who conducted the home study. The field defaults to the name of the  worker who is currently logged on. This field is required.

Provider Type

Select the provider type. This list displays the types that are active for this provider.

Level of Care

Select the level of care. This field is required for home providers, who are licensed or certified for a specific level of care.

Start Date

Enter the date on which the home study was started. Use the format MM/DD/YYYY.

This date must be the same as or after the effective date on the provider type record. If a previous home study exists for this provider type record, the start date on this record must be later than the start date on the previous record.

You cannot enter a date in the future.

Priority

Select the priority of this home study.
