Agency
Select the agency(ies) to include in this report, then click Add>. The agency(ies) you selected appear in the Selected list box.
Inquiry Type
Select the type of inquiry to include in the report.
Inquiry Received by Worker
Select the worker whose inquiries you are including in this report. This list displays all inquiry workers.
Decision
To include inquiries of with a particular decision in the report, select the decision.
From Inquiry Date
Select the first inquiry date to include in the report.
To Inquiry Date
Select the last inquiry date to include in the report.
Inquiry ID
To generate a report for a certain inquiry, enter the inquiry ID number.
