From Date

Enter the earliest date you want the report to cover. This is a required field.

To Date

Enter the latest date you want the report to cover. (To generate a report for one day, enter the same date in both date fields.) This is a required field.

Agency Unit

Leave this field blank to generate a report that covers all units in the agency. Otherwise, select a unit from the list.

Supervisor

Leave this field blank to generate a report that covers all supervisors in the agency or selected unit. Otherwise, select a supervisor from the list.

Employee

Leave this field blank to generate a report that covers all employees within the scope defined by the Agency Unit and Supervisor fields. Otherwise, select an employee from the list.
