The provider activity log reflects the activities--point in time occurrences, that are associated with the provider, provider members, or children placed with the provider.
You can print a provider's activity log as follows:
From the Home screen, click the Provider tab.
Click the Workload tab. The Workload screen appears.
Click the Select link for the provider who's record you want to view. The Provider Overview screen appears.
Click Activity Log in the navigation bar on the left. The Activity Log Filter Criteria screen appears.
Enter your filter criteria. You can display activity records within a certain date range, by a certain worker, of a certain category, of a certain contact type, and so on.
Click Filter. The activity records that met your criteria appear in the Activity Log table. Restricted activity records are not included in the table if you are not assigned to the provider record.
For a single activity, click the Report link for the activity you want to print.
To print multiple records, click Generate Report button. The Document Details screen appears. It lists each time this record was generated and saved to the Ohio SACWIS database. To print an existing activity log report, click the ID link.
For a new report, click Generate Report. The report appears in a PDF.
To print the report, select File, then Print in your browser menu.
To save the report to the report history in Ohio SACWIS, click the Save button in the lower-left area of the screen. Or click Cancel if you do not want to save the report.
If you opened an existing report, close the browser window to return to the previous screen.
