View Agency Certifications

Agency information is maintained by ODJFS. View an agency's certification records as follows:

1. From the Home screen, click the Provider tab.

2. Click the Agency Certifications tab. The Agency Search Criteria screen appears.

3. Select Agency Type, the Agency and then Certifying Agency.

4. Click Search. The results display in the Agency Certification Results and Maintain Agency Certification(s) tables.

5. Click the View link. The Approval/Certification Information screen appears.

6. Click Close to return to the Agency Search Criteria screen.
