Responsible party details

Name

Select the name of the person who will be responsible for the safety plan activity you selected on the previous screen.

This list displays active case members and associated persons. You can click Person Search, then locate and select another person. After saving the screen, the person you selected is added to the safety plan and appears in participant lists

Phone

Enter the 10-digit telephone number of the person whose name you selected in the previous field.

Enter any additional details about this person's responsibility. You can enter up to 200 characters.
