Actions Taken by Agency (check boxes)

Select the check box next to each action the agency took to address this PSA. If you select the first check box, then select Yes or No to answer the question underneath that check box.

If Other, Describe

If you selected the Other check box, enter a description of the action. You can enter up to 1,000 characters.

Case Situation (check boxes)

Select the check box next to each statement that applies to this case.

Date

If you selected the check box above a date field, enter the date on which the actions were taken. Use the format MM/DD/YYYY.

Violation

If the children were taken in violation of a court order, select that check box, then enter a description of the violation. You can enter up to 1,000 characters. This field is required if you selected the check box above it.

If Other, Explain

If you selected the Other check box, enter a description of the situation. You can enter up to 1,000 characters. This field is required if you selected this check box.

Supporting Documents (check boxes)

Select the check box next to each document the agency can provide to support the PSA, if any.

Additional Information

If the agency has more information, select this check box, then enter a description of that information. You can enter up to 1,000 characters. You must enter a description if you selected the check box.

Method Document Was Sent

Select the method by which the PSA was sent.

Date sent

Enter the date on which documents were sent. Use the format MM/DD/YYYY.  This field is required if you selected one of the check boxes at left.
