Characteristics

Provider Type

Select the type of provider this is. You must choose the same provider type that's in the provider's information record. This field is required.

Characteristics Status

Select the status of this characteristics record. The default is "In Progress." This field is required.

For a home or residential provider, the system completes this field automatically based on the home study or licensing process. An acceptance criteria record cannot be approved until a placement criteria record has been added.

Effective Date

Enter the date this information is in effect. Use the format MM/DD/YYYY. For a home or residential provider, the system automatically applies the approval date of the home study or license/certification. This field is required.

End Date

Enter an end date to close this characteristics record. Use the format MM/DD/YYYY. The system automatically enters an end date when a new characteristics record with the same group is approved.

Select Group

Select the kind of child characteristic you are defining for this provider. This field is required. Click the Show button to list the details of this group in the table below.

Willing to Consider

Select this check box to indicate that  this provider will consider taking children with the characteristic listed in the Description column.

Unwilling to  Consider

Select this check box to indicate that this provider will not consider taking children with the characteristic listed in the Description column.
