Agency Type

Select the type agency included in the report.

Agency Available

Select the agency(ies) to include in the report, then click Add>>.

Agency Selected

This field displays the agency(ies) selected from the previous field.

Provider Type Available

Select the home provider type(s) to include in the report, then click Add>>.

Provider Selected

This field displays the type(s) selected from the previous list.

County Available

Select the county(ies) to include in the report., then click Add>>.

County Selected

This field displays the county(ies) selected from the previous field.

From Date

Enter the beginning date of the range when provider records were in effect. Use the format MM/DD/YYYY.

To Date

Enter the ending date of the range when provider records were in effect. Use the format MM/DD/YYYY.
