Agency Type

Select the type of agency to be included in the report.

Agency Available

Select the agency(ies) to include in the report, then click Add>>.

Agency Selected

This field displays the agency(ies) selected from the previous field.

Provider ID

To generate a report for a specific provider, enter the provider's system-generated ID number. Do not enter any other parameters.

Provider Type

Select the type of provider to include in the report.

From Refusal Date

Enter the beginning date of the range when the provider refused placements. Use the format MM/DD/YYYY.

To Refusal Date

Enter the ending date of the range when the provider refused placements. Use the format MM/DD/YYYY.
